Covid-19 Liability Forms
Due to the 2019-2020 outbreak of the novel Coronavirus, COVID-19, we are taking extra precautions with the intake of each client, we require this form to be signed before treatments. Can be printed / signed at home and bring with you, or filled out prior to treatment.

Late Arrivals

Appointment times have been arranged specifically for you and our other clients. Unfortunately, we cannot provide late arrivals with an extension of their appointment by bumping other clients.
In special cases we may be able to accommodate a partial or full appointment. This will be at our discretion and only with proper advanced notification of your arrival.
Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session

No-Show & CancellationPolicy

Please keep your commitment and be considerate of others when you book your appointment. This is time that is set aside specifically for you. Missed appointments prevent us from accommodating our other clients and as a small business, we only get paid when we deliver a service. Therefore, we request that you call at least 5 hours in advance to cancel or reschedule. Please Note: (If Booked online, you can cancel or reschedule threw your booking account) When you are unable to keep your appointment and we receive no notification, you will be required to pay 50% cost of the treatment as booked. Please note that if you simply do not show up to your appointment, or if you cancel at very short notice, your therapist may also require pre-payment of future appointments and/or have you keep a credit card on file.
If an emergency situation arises, please let us know so that we can treat your specific situation with personal attention. We recognize that there are circumstances that are out of your control (sudden illness, family emergencies, etc.) and your therapist may make an exception to the above policies on those rare occasions.

By booking an appointment either online by phone or otherwise you authorize us to charge your credit card on file for any cancellation fees unless other arrangements are made. Clients who no-show or late cancel more than once will be required to pay for their future appointments at the time of booking.

Gift certificates - may also be forfeited for cancellation fees with less than 24 hours notice. Gift certificates will be forfeited for no shows. Missed appointment fees are not covered by any insurance plan.
